When to Use Activity Forms, Certificates of Liability Insurance and Waivers

Troop and Service Unit Activity Forms

These forms need to be completed by a troop leader or service unit representative when the troop or service unit is participating in a trip or event and when a troop is going somewhere other than their usual meeting place and time. Troop Trip Activity forms are needed for all council events. Activity Forms need to be submitted two weeks prior to the activity. Activity forms may be submitted via the internet by choosing the internet option in the forms and resouce library.

Certificates of Liability Insurance

Certificates of Liability Insurance need to be on file in the Waltham office for any activity featured in Safety-Wise as requiring it. This requirement is generally listed under the Planning and Supervision or the Site section of a checkpoint in Safety-Wise. Examples of these activities would be hayrides, amusement parks, gymnastics, and horseback riding. It is also council policy to request a certificate of liablity insurance when risk is involved in the activity such as rock wall climbing, white water rafting, and martial arts.

Check our list of certificates of liability already filed or call the Waltham office and speak with the receptionist or the Manager of Volunteer Administration.

Certificates of liability insurance need to be renewed annually as they expire at different times of the year. Certificates are only requested when a troop asks to go to the site. So the first troop to go to that site in a year will be the one to ask for the certificate.

Waivers

Often times facilities that offer activities that may involve risk will require you to sign a waiver. It is a GSUSA policy that we do not sign waivers which give away your rights. However, for some activities, GSPTC will allow you to sign waivers after we have screened them and have agreed to accept the liability. The council has final say on accepting liability. We will make every effort for girls to try new opportunities, but ultimately the goal is to keep them safe and to protect their rights.

Extra Insurance

Extra activity insurance is needed anytime you are holding a Girl Scout event, trip, or function that will include non-scouts. It is also needed for trips that are more than three nights, international trips and where girls do not have accident and sickness insurance.
